ADOPTION AGREEMENT
READ THIS AGREEMENT CAREFULLY BEFORE SIGNING AS IT CONTAINS IMPORTANT PROVISIONS REGARDING YOUR RIGHTS AND RESPONSIBILITIES.

This agreement is made and entered into this day, by and between FAITH & HOPE FOUNDATION, Inc. and the following individuals, hereinafter referenced to as "Adopter".

In consideration for receiving the cat described herein, I/we expressly covenant and agree to the following:
I/we have never been charged with cruelty to animals.

Adopter agrees that if they are no longer able to care for the animal they have adopted, they will contact Faith and Hope Foundation to assist with possible options.

I/we agree that copies of the medical records from the first vet visit after adoption will be furnished to Faith & Hope Foundation.

I/we agree that the pet will not be abused in any manner. It will not be permitted off the property of the Adopter unless accompanied by a mature, responsible individual. The pet will not be the subject of, nor subjected to any biological, chemical, psychological or other experiments, nor shall the pet be subjected to cruel means of training.

Adopter expressly covenants and agrees that the adopted cat shall not be subjected to any form of cosmetic surgery or alteration including, but not limited to, tail docking, claw removal, removal of rescue microchip etc., as such procedures are extremely painful for the cat and do not provide the cat with any actual benefit.

Adopter recognizes that, although Faith & Hope has made every possible effort to provide a pet which is compatible with the Adopter's home environment, Faith & Hope does not warrant the temperament or behavior of the pet and will not be liable for any acts of the pet while living with Adopter, nor for any costs, expenses or damages incurred as a result of the pet's conduct.

Adopter will keep the pet for so long as it shall live and will not transfer the ownership or custody of the pet to any other person or to any business or organization other than for purposes of veterinary treatment and/or temporary lodging of the animal while Adopter is temporarily away.

Adopter agrees to comply with all state and local laws and ordinances related to the keeping of this pet, including licensing.

Adopter will see that the pet wears a collar and ID tags at all times. The pet has been micro-chipped by Faith & Hope, and no attempt shall be made by the Adopter to transfer the microchip to another party, or alter the chip in any manner with the exception of adding the Adopter’s name to the chip registration. The Adopter agrees to keep SAFE listed on the microchip registration.

This pet is presumed to be in healthy condition. Faith & Hope will not be responsible for any condition for which clinical symptoms are not in evidence at the time of adoption.

The adoption fee is non-refundable.

Adopter accepts possession of the pet described above at Adopter's own risk and hereby expressly releases and waives any claim or cause of action against Faith & Hope, which Adopter may have now or in the future for any damages to person or property caused by said pet.

In the event of a dispute arising out of the terms and conditions of this Adoption Contract, and in the event that Faith & Hope prevails in any legal proceedings initiated as a result of said dispute, then Adopter agrees to reimburse Faith & Hope for its reasonable attorney's fees and expenses incurred as a result of the dispute.

When applicable, Adopter agree to have the pet spayed/neutered at the age recommended by your veterinarian and will provide Faith & Hope with proof of such spaying/neutering.
